• Choices when going home from a hospital or a skilled nursing/rehabilitation facility
• Pros and cons of what it looks like to be cared for at home

This program is presented in partnership with Home Healthcare, Hospice and Community Services (HCS). HCS is a 501(c)(3) not-for-profit organization serving over 2000 people in 52 southwestern New Hampshire communities every day. HCS is Medicare certified and licensed by the State of New Hampshire as a home care provider.
